Overview:
I got it to work eventually but it took a while to figure out and I can see why a lot of the reviews say it doesn't work. The product would be fine if the seller provided detailed instructions, but they don't so you have to figure out yourself. I came close to giving up a couple times so I thought I'd write up what I learned to try to help other people who buy this.
Use Case:
I purchased this device so I could transfer my wife's SIM card contacts from her old SIM card to her new SIM card. Her old phone had a full size SIM and the new one has a smaller size SIM. We have cell phone service from an MVNO so we can't just go to a store and ask them to transfer the contacts from one SIM to another.
My attempt at a User Guide:
When you order this product you will receive a tiny cardboard box with the SIM card reader inside. There will also be a small piece of folded up paper in the cardboard box. I did not see the paper at first so I was really lost until I saw another review mention it! Here is what the paper says (I can't include the URL per Amazon's review policy):

Here is what I did to install and use this product on Windows 7. The link on the piece of paper takes you to DropBox. Once you navigate to this page you will see a bunch of software for various products. You need the one that says 'COTHSIMRDX01 [SIM Card Reader , Blue]' (see picture). Once you click that link it will take you to a new page listing a single item 'Usb-SIM9.0.zip'. Click 'Usb-SIM9.0.zip' then click 'Download' in the upper right and then click 'Direct download'. Save the file to your device and make sure to note where you save it.
Double-click the Usb-SIM9.0.zip file to open it. You should see 9 files. Run Setup.exe to install the software you need to backup and restore your SIM card data. The install process is straight-forward. I just accepted the defaults and clicked Next for everything. When it finishes, there will be three utilities installed: SIM Data, SIM Editor, SIM Scanner. There are installed under All Programs > GSM SIM Utility V9.0. The only one you need to be able to backup and restore your SIM Card contacts is SIM Editor.
Insert your SIM card and then try to connect. There are icons at the top of the window. Click the far left one to connect. The other buttons you will need are the 'Save to file' and 'Open from file' button and the 'Read from SIM card' and 'Write to SIM card' buttons. You can also switch between 'Phonebook' and 'SMS' (text messages) via the buttons that are second and third from the left.
Troubleshooting:
There is one other file in Usb-SIM9.0.zip that may be of interest: PL2303_Prolific_DriverInstallers_v1.8.0.exe. When you plug in the SIM card, Windows 7 should automatically figure out what drivers you need and install them. If that doesn't work, then running PL2303_Prolific_DriverInstallers_v1.8.0.exe may be necessary.
I had no issues getting the software to connect to the full size SIM card but the smaller SIM card took some time. I had to put the smaller SIM card onto a full size SIM card adapter to get it to fit correctly. It seemed to fit fine but I couldn't connect to it. I tried pulling it very slightly and then tugging on it from one side and that allowed it to connect.
